Step 1: Complete Your Assessment in Your Home State

You must get an alcohol/substance use assessment from a licensed provider in your home state, not North Carolina.

It must include:
  • Full clinical screening
  • Diagnostic impression
  • Recommendation for education or treatment

Step 2: Complete All Recommended Classes in Your Home State

Based on your assessment, you’ll complete:
  • ADETS / Alcohol Education, OR
  • Treatment hours (20–Inpatient depending on your assessment)
You do all of this locally, in your state, not with us.

Step 3: Gather Your Documentation

You will need:
  • Assessment summary
  • Proof of completion
  • Attendance records
  • Discharge or completion letter
  • Any course certificates
If you’re not sure what you need, we will tell you exactly what to request.

Step 4: Send All Documents to Breathe – A Center for Wellness

This is where we come in.

We will:
  • Review everything
  • Make sure it meets North Carolina standards
  • Request corrections if needed
  • Prepare your paperwork
We specialize in making sure your documents meet NC requirements the first time.

Step 5: We Submit Your 508 to North Carolina for You

Once all documents are approved, we submit your 508 directly to the North Carolina DMV/DWI Services.

The 508 tells NC:
  • You completed your assessment
  • You completed all recommended hours
  • You met the state requirements
This is often the step out-of-state clients are missing.

Step 6: Follow Up With Your Home State DMV

After NC updates your record, your home state can finalize your reinstatement.
